First Patch not sending program change

Hi,

Operating System:
====================
Mac OS Mojave

Description
===========
I have a serious issue as the first patch does not send a Program Change. Since Piano is used in 95% of the songs, when I change song, the sound doesn't change.

Steps to reproduce
------------------
Just try a patch change to the first patch (Piano 1) on QMIDI and you will notice that it only sends bank select (cc 0 and CC 32) and not the program change. This doesn't happen on any other patch.

Can you provide a MIDI file with this issue?

I've tried with this file:
piano.mid
and it seems ok:

Actually I can reproduce the issue without a midi file.

If I select Piano 2 From Sc55 or 88 patch in the mixer I see in the midi monitor that the control changes for the bank get sent but not the program change.
